Have you been told you need to see a dental specialist?

Your First Visit

At your first visit to Dental Specialty Associates, you can expect to be asked a number of questions about your dental and overall health history and problems. To make the visit as productive and efficient as possible, please bring the following information:

  • Any health or dental insurance cards you may have.
  • Any x-rays you have been given.
  • A complete list of medications you are currently taking.
  • Any referral slips from other healthcare providers.
  • A credit (we take all major credit cards) or debit card.

The staff is very professional taking the time explain the procedure and options. I would recommend them for any dental needs.
